Comprehensive Community Support Services (CCSS)

Comprehensive Community Support Services (CCSS) -  The purpose of CCSS is to coordinate and provide services and resources to children, adolescents and their families as necessary to promote recovery, rehabilitation, and resiliency. CCSS identifies and addresses the barriers that impede the development of skills necessary for independent functioning in the community, as well as strengths which may aid the client or family in the recovery or resiliency process. CCSS activities address specific goals in the following areas: independent living, learning, socializing, recreation, and school.

 
 

CCSS also includes supporting a client and family in crisis situations and providing individual interventions to develop or enhance the client's and family's ability to make informed decisions and independent choices.

Target Population

  • Children or adolescents experiencing or at risk of serious emotional/neurobiological /behavioral disorders
  • Children or adolescents with chronic substance abuse problems
  • Children or adolescents with co-occurring dis-order (mental illness/substance abuse)

Assessment Required - In order to recieve this service, a client must undergo a behavioral assessment that results in a Service Plan.

Services
Assistance is provided to the client and family in the development of a service plan including a recovery/resiliency management plan. Assessment, support, and intervention in crisis situations including the development of a crisis management plan which recognizes the signs of crisis/relapse, use of natural supports, and use of alternatives to higher levels of care is provided (if needed).
